Customer Guide for Workplace Injury
In the event a worker is injured at your work location while on a shift, the worker should report the injury to their supervisor immediately or as soon as is practical.
Upon being notified of the injury, the supervisor should:
Assess the need for medical attention. If the worker needs urgent care, call 911 or an ambulance to take them to the hospital.
Take photos of the injury, the work area, equipment, etc., before anything is disturbed.
Contact your WorkWhile Account Manager to notify them of the injury.
Instruct the injured worker to notify WorkWhile of the injury through the app and complete the employee Workers’ Compensation Claim Packet that will be electronically sent to them.
Complete the Supervisor Investigation Report and, if applicable, have all witnesses complete the Witness Statement form. These forms will be electronically sent to you and the witnesses by WorkWhile but you can complete them and send back manually if you prefer
Send all forms and photos to your WorkWhile Account Manager and/or submit the electronic documents
